We’re creating videos to help other teens feel seen, understood, and empowered to make positive choices. 

If you are selected to be a youth ambassador, you can expect to schedule a full- or half-day video filming session and photoshoot with our creative team. You'll be asked about your passions, relationships, and life experiences, and be given a chance to share your personal story. 

This form helps us get to know you so we can plan a video that feels true to who you are. Please answer honestly.
